Charts Audit and Clinical Review Process

Consistent with TNS’s commitment to quality patient care, Therapy Network Solutions has established medical record standards to ensure efficient, effective and complete clinic documentation practices. This is done via quarterly chart audits. A sample of active and closed charts is reviewed quarterly by our expert Physical Therapist, Clinical Review Specialist to ensure that established policies are followed in providing services.

In addition, an in house clinical review is done, by our expert Physical Therapist, Clinical Review Specialist, on certain charts that have been triggered due to a high visit count.
This process monitors therapy utilization through chart audits and peer to peer communication that identifies skilled care, medical necessity, evidence based practice, lack of progress or compliance for each case treated. This provides both quality and cost monitoring for our customers.

Guideline Application for Clinical Review

All applicable charts are reviewed at the 10th visit to establish a visit count guideline based on the specific injury and clinical benchmarks.
Reviewer will review all pertinent information including notes, evaluation and re-evaluations, diagnosis, co-morbidities, goals, progress, and clinical practice guidelines.
If necessary a discussion will be held with the treating therapist(s) to obtain any further information.

Clinical Review Recommendation

When a review is complete, a ‘Clinical Review Summary’ is completed, including the reviewer’s clinical recommendation. This will be filed in the patient’s chart.
